Once signed up for an Ethic Indoor program (unless canceled by Ethic Indoor), there are no cash, check, or credit card refunds. Ethic Indoor will offer a full in-house credit for personal instruction, future programs, and/or membership at OAW Indoor Sports Complex.

Spectator Waiver Agreement

If tax exempt, please send the director your tax exempt information. If paying by check, please disregard the processing fee (3%). Checks can be made out to Ethic Indoor, LLC & mailed to: OAW Indoor Sports Complex 5330 S Racine Ave New Berlin, WI 53146
